Primaris leads healthcare quality improvement initiatives for the Medicare program in Missouri as a subcontractor to TMF Quality Innovation Network-Quality Improvement Organization (QIN-QIO). As part of the TMF QIN-QIO, Primaris works with Missouri providers to promote patient-centered care, make care safer and more affordable, and improve population health. These initiatives include:

  • Reducing readmissions,
  • Improving hospital and physician reporting,
  • Improving diabetes self-management,
  • Improving cardiac health,
  • Improving sepsis,
  • Improving nursing home care,
  • Reducing medication harm, and
  • Improving prevention coordination through HIT.

Quality Payment Program (QPP) and Small, Underserved and Rural Support (SURS)

As a subcontractor to TMF Health Quality Institute, Primaris works with eligible physician practices and clinicians to help them transition to the Merit-based Incentive Payment System (MIPS) and successfully advance through the program’s performance categories by providing technical assistance, education and outreach. MIPS is one of two payment paths under the Centers for Medicare & Medicaid Services’ (CMS) Quality Payment Program (QPP).

Primaris works to provide QPP assistance to clinicians throughout the state in small (15 or fewer clinicians), rural and underserved practices and with practices with 16 or more clinicians. As part of the TMF team, Primaris will provide customized technical assistance to MIPS-eligible clinicians, which may include:

  • Evaluating practice readiness for participating in a new payment model
  • Engaging practices in continuous quality improvement
  • Assessing and optimizing Health Information Technology
  • Supporting change management and strategic planning
  • Developing and disseminating education and training materials
  • Enabling peer-to-peer learning and local partnerships
