When the Institute of Medicine (IOM) released the historic report To Err is Human, healthcare professionals around the country became acutely aware of the need to examine the issue of patient safety in every aspect of healthcare. The IOM report, published in 1998, estimated as many as 100,000 patients die each year in US hospitals because of errors in medical care. Since 1998, much has been done to improve patient safety by the healthcare industry. Extensive research to determine how and why errors in medical care occur and safer practices have emerged as a result of this national effort to improve patient safety.

Primaris has been working to improve safety and quality in healthcare practices since 1999. We are committed to continuously working with our partners in healthcare to improve the safety of Missourians when they need hospital and other healthcare services.
