Discharge to Community
Improvement in the discharge to community outcome is demonstrated by an increase in the percentage of patients who are able to live in the community at discharge. Higher percentages are better.
Patients who are transferred to a hospital, assisted living, or nursing home are not able to remain in the community. To remain the community, patients must be able to care for themselves and maintain their medical regime or there must be a willing and able caregiver present. In the absence of these skills or a caregiver, the patient may be placed into a chronic care facility or be readmitted to a hospital. If the patient is able to remain in the community, they are more independent, self-confident and active.
