Diabetes


Adult onset diabetes is common in the Medicare population, with over 190,000 Americans dying each year from the disease's complications. For most patients, complications from diabetes (including blindness, kidney failure, nerve damage and cardiovascular disease) can be prevented or delayed with appropriate monitoring and treatment. However, studies indicate that diabetes care is sub-optimal.
Primaris aims to reduce the rates of blindness, amputations, kidney failure and diabetes-associated cardiovascular disease by providing proven-effective tools and resources for Missouri health care professionals.
